funny thing I found regarding health and safety warnings. Essentially one being put up means that somewhere an injury occurred because of the thing they now warn about.

Lots of Darwin Awards candidates out there for sure!

funny thing I found regarding health and safety warnings. Essentially one being put up means that somewhere an injury occurred because of the thing they now warn about.

Many years ago, one of my clients had four nails ripped open using one of our(IBM) machines! Almost severed the ends of his fingers!
I submitted a safety report and suggestion to management. It was rejected! Reason? There was no clear place to post a warning label! With a poorly placed label, the company would be admitting knowledge of the danger but didn't take the proper steps to prevent injury! This would increase their liability! Since the units were approaching EOL, it was cheaper to just handle the accidents on a single basis!
Well, later that year, I took an assignment in the plant. While there, an employee was injured the same way! I got my manager involved, and he demanded they pay me for my suggestion report ($$$)! Still no labels, instead, they pulled the unit from sales! I received checks for two years!

This second link even mentions someone else who bought a puppy, only to discover they had a fox... but again, doesn't compare to the tibetan mastiff pup that grew into a bear...

I don't know how they couldn't realize their mistake, but perhaps it was a good thing after all, as the cub could have ended up in worse hands...
